Lower Body Lift (Belt Lipectomy)

What Is a Lower Body Lift?

A lower body lift, also known as a belt lipectomy or circumferential body lift, is a comprehensive body-contouring procedure designed to remove excess skin and tissue from around the entire lower torso.

The procedure commonly addresses loose or hanging tissue involving the:

  • Abdomen
  • Flanks and waist
  • Lower back
  • Buttocks
  • Lateral thighs

A lower body lift is most commonly performed after substantial weight loss, including weight loss following bariatric surgery, medical weight-loss treatment, or significant lifestyle change. It may also be appropriate for selected patients with extensive skin laxity related to aging, pregnancy, genetics, or repeated weight fluctuations.

Unlike a standard or extended tummy tuck, which primarily treats the abdomen and flanks, a lower body lift uses an incision that extends circumferentially around the lower torso. This allows the surgeon to remove excess tissue from both the front and back of the body while improving the contour of the waist, buttocks, and outer thighs.

The procedure typically includes an abdominoplasty component in the front and lifting or tightening of the lower back, buttocks, and lateral thighs in the back. When abdominal wall laxity or rectus diastasis is present, repair of the abdominal fascia may also be performed.

During surgery, the patient may be carefully repositioned so the anterior and posterior portions of the body can be treated safely.

In selected patients, the patient’s own tissue may be preserved or rearranged to improve buttock shape and projection. The exact technique depends on anatomy, tissue quality, and the patient’s goals.

The principal trade-off of a lower body lift is a long, circumferential scar in exchange for removal of extensive loose skin and a more complete improvement in lower-body contour.

Who Is It For?

A lower body lift may be appropriate for patients who:

  • Have loose, hanging, or circumferential skin around the abdomen, flanks, lower back, and buttocks
  • Have experienced significant weight loss
  • Have skin laxity that cannot be adequately corrected with a standard or extended tummy tuck
  • Have deflation or descent of the buttocks and lateral thighs
  • Experience recurrent skin irritation, rashes, chafing, or hygiene difficulty within skin folds
  • Experience discomfort, heaviness, or pulling from excess skin
  • Have difficulty exercising, walking comfortably, or finding properly fitting clothing because of skin excess
  • Are at or near a stable, sustainable weight
  • Are in good overall health
  • Have adequate nutritional status
  • Do not smoke or use nicotine, or are able to discontinue nicotine as directed before and after surgery
  • Understand and accept the location and length of the circumferential scar
  • Have realistic expectations regarding surgery, recovery, and scar quality

A lower body lift is not a weight-loss procedure. Patients should generally complete the majority of their weight loss and maintain a stable weight before surgery.

Lower Body Lift vs. Extended Tummy Tuck

An extended tummy tuck treats the abdomen and carries the incision farther around the sides to address skin laxity of the flanks and lateral waist.

A lower body lift extends around the entire torso and addresses not only the abdomen and flanks but also the lower back, buttocks, and lateral thighs.

Patients with primarily anterior and lateral laxity may be candidates for an extended tummy tuck. Patients with significant circumferential laxity and buttock or lateral-thigh descent may benefit more from a lower body lift.

The appropriate operation depends on the location and extent of skin excess, previous weight loss, scar tolerance, and individual goals.

Preparation

Because a lower body lift is an extensive procedure, careful preoperative preparation is essential.

Preparation may include:

  • A detailed medical history and physical examination
  • Preoperative laboratory testing
  • Medical or cardiac clearance when appropriate
  • Maintaining a stable weight for several months
  • Discontinuing nicotine before and after surgery according to your surgeon’s instructions
  • Reviewing all prescription medications, over-the-counter medications, vitamins, supplements, and weight-loss medications
  • Temporarily discontinuing medications or supplements that may increase surgical risk when medically appropriate
  • Ensuring adequate protein and overall nutritional intake
  • Arranging assistance at home during the early recovery period

Patients who have undergone bariatric surgery or substantial weight loss may require additional nutritional assessment. Testing may include evaluation for anemia, iron deficiency, vitamin deficiencies, low protein levels, or other conditions that can impair healing.

Specific fasting and medication instructions will be provided by the surgical and anesthesia team.

Operative time varies according to the amount of tissue removed, the surgical technique, whether abdominal wall repair, liposuction, or buttock auto augmentation is performed, and whether other procedures are combined.

Because of the extensive nature of the procedure an overnight stay is usually required.

Additional Information

Temporary effects may include:

  • Significant swelling
  • Bruising
  • Tightness around the waist and lower torso
  • Soreness or discomfort
  • Temporary numbness or altered skin sensation
  • Fatigue
  • Temporary drainage from surgical drains
  • Difficulty standing fully upright during early recovery
  • Temporary asymmetry related to swelling

Swelling may persist for several months and gradually improve as healing progresses.

Potential risks include:

  • Bleeding or hematoma
  • Infection
  • Seroma or fluid accumulation
  • Delayed wound healing
  • Wound separation or breakdown
  • Skin or fat necrosis
  • Unfavorable, widened, or asymmetric scarring
  • Persistent numbness or altered sensation
  • Contour irregularity or asymmetry
  • Buttock flattening or contour changes
  • Deep vein thrombosis
  • Pulmonary embolism
  • Anesthesia-related complications
  • Need for wound care, scar revision, or additional surgery

Because the incision extends around the torso, wound-healing problems may occur in areas exposed to greater tension, moisture, pressure, or movement, including the posterior midline and lateral portions of the incision.

Risks can be reduced through careful patient selection, nutritional optimization, nicotine avoidance, early postoperative mobility, appropriate blood-clot prevention, and adherence to postoperative instructions, but they cannot be eliminated.

Recovery after a lower body lift is more extensive than recovery after a standard tummy tuck because both the front and back of the lower torso are treated.

Patients are encouraged to begin gentle, assisted walking soon after surgery to promote circulation and reduce the risk of blood clots.

During early recovery, patients may walk slightly flexed because of abdominal tightness. They gradually return to a more upright posture as healing progresses.

Patients should avoid prolonged pressure directly on healing incisions and should follow individualized instructions regarding sleeping and positioning. Depending on the surgical plan, patients may be advised to sleep on their back with the torso and knees slightly flexed, on their sides, or in another supported position.

Compression garments are commonly recommended to provide support and help manage swelling.

Many patients require approximately two to four weeks before returning to desk-based work. Patients whose jobs require lifting, prolonged standing, bending, or physical labor may need additional recovery time.

Heavy lifting, strenuous exercise, deep bending, and activities that place significant tension on the incisions should be avoided during early healing and resumed gradually according to the surgeon’s instructions.

Pain is managed with an individualized postoperative pain-control regimen. Early gentle walking is encouraged, but activity should be increased slowly.

Drains, when used, are removed once fluid output has decreased to an appropriate level.

A lower body lift can provide:

  • Removal of circumferential loose or hanging skin
  • A flatter and firmer abdomen
  • Improved definition of the waist
  • Smoother contour of the flanks and lower back
  • Elevation and improved contour of the buttocks
  • Improvement in lateral-thigh laxity
  • Reduced skin-fold irritation, chafing, and hygiene difficulty
  • Improved comfort and clothing fit

A significant improvement in contour is visible soon after surgery but swelling initially obscures the final result. The body continues to change over several months as swelling resolves and tissues settle.

The circumferential scar is permanent. Every effort is made to position it where it can be concealed beneath underwear or swimwear when anatomy allows. Scar quality and position vary according to the amount of tissue removed, skin quality, tension, healing characteristics, and postoperative care.

Scars typically fade and soften over time, but maturation may continue for 12 to 18 months or longer.

Results are generally long-lasting when a stable weight and healthy lifestyle are maintained. Significant weight gain, weight loss, pregnancy, and aging may stretch the tissues and alter the surgical result.

Individual results vary according to the degree of skin laxity, tissue quality, weight-loss history, nutritional status, scar formation, and overall health.
