Applications for Associate Membership

Thank you for applying for Associate Membership in ISAPS. Membership is at the invitation of the Society. The following information will help you determine your eligibility.

Use the menu to complete the on-line application:

  1. Read the requirements carefully and make sure you have everything you need before starting.
  2. Complete the on-line form: personal and demographic data, training data, photo upload, membership certificate upload, applicant´s certification and sponsor selection.
  3. Click on SUBMIT ON-LINE APPLICATION only when all the information is ready to be sent.
  4. Click on PAY APPLICATION FEES after the online form has been submitted.

Requirements

  • Applicants MUST be in training in an official training program of the ISAPS-approved National Society of Plastic Surgery in their country, or in practice but not yet eligible for Active Membership in their own ISAPS-approved National Society.
  • Associate Members may attend membership meetings but may not vote, hold office, or serve on ISAPS committees.
  • It is the responsibility of Associate Member to apply for Active Membership when they become eligible.
  • Associate Members who have not become Active Members of ISAPS according to the application procedures for Active Membership shall have their Associate Membership automatically terminated.
  • All applicants agree to comply with the ISAPS Code of Ethics.
  • The APPLICANT is responsible for completing the online application Package.
  • Incomplete Applications will not be accepted.
  • All applicants' names approved by the Executive Committee must be also approved at the a full member voting. Notice of the applicant's acceptance will be sent when the vote is completed.

You must adjust several items during the online application. We recommend you to make sure you have everything before starting the process. DO NOT SEND ANYTHING NOT REQUESTED.

  • 1. Certificate or letter confirming your CURRENT enrollment in an official training program of the ISAPS- approved National Society of Plastic Surgery in your country (PDF FORMAT). If you cannot upload the certification as PDF format, you can send it by email (isaps@conmx.net) or by fax (1-603-643-2325).
  • 2. An electronic photo suitable for publication in ISAPS membership roster on the ISAPS website (JPEG or TIFF formats).
  • 3. During the online process, you should also select 3 sponsors who are current active or life ISAPS members, at least one from applicant's own country. You can first check the list of the ISAPS membership, here. If there are no ISAPS members in the applicant's country, then members from another country may sponsor the applicant.

I certify that the above information is truthful, and acknowledge that any false statements may result in the rejection of this application or subsequent loss of membership. I also understand that I will not be considered by the Membership Committee without full payment of the application fee (non-refundable) and the annual dues for the first year (to be refunded if my application is unsuccessful). Further, I agree to comply with the code of ethics, specific principles, and bylaws of ISAPS.

4. Sponsors

You have to select 2,3 or 4 sponsors who are current active or life ISAPS members, at least one from applicant's own country. If there are no ISAPS members in the applicant's country, then members from another country may sponsor the applicant.

National Secretary should not be one of your sponsors.
